You provided a great example of a patient with PCOS diagnosis and how it needed multiple visits in order to properly diagnose her. You mentioned how treatments/ referrals were consistent with guidelines.

Application to Practice: As student stated in her discussion of her PCOS patient in clinical practice, it is clear that PCOS could be easily missed. 1) PCOS presents in many different ways in each patient and other factors contributing to difficulties in menstruation/ pregnancy can be missed as far as hormonal issues. Thyroid disease, hyperprolactinemia, and nonclassic congenital adrenal hyperplasia (primarily 21-hydroxylase deficiency by serum 17-hydroxyprogesterone [17-OHP]) (Table 3)- these are all conditions that mimic PCOS and need to be eliminated from differentials. 2) Insulin problems can also be easily missed if the provider is only focused on one aspect of the patient care and not considering all possibilities. This clinical practice guideline ensures that this diagnosis is not missed and is also treated/managed properly. Proper treatment of PCOS with OSA helps improve insulin sensitivity. 3) My specific rotation sees a lot of women’s health, so this will support any future differential diagnosis thought process and management. Knowing these guidelines, I now know metformin can be useful in not only helping insulin sensitivity but can also synergistically help fertility. |

Slides were well organized, but I think you would benefit in making more specific slides that denote rubric check off points, ie: a pathophysiology slide so you don’t forget it, etc. Narration was understandable and paced well. I could tell you sped up at the end as you were getting close to the 15 minute mark. You stayed under the 15 minute mark. References were noted. |
