PATIENT SCENARIO

Lucy is a 42 year old Caucasian female being seen in the office for recent onset of weakness, fatigue, feeling faint and loss of appetite with intermittent nausea, vomiting and abdominal pain.  Lucy reports unintentional weight loss of 15 lbs over the last two months.  She is also concerned with “dark spots” that seem to be coming up on her elbows and knuckles and decreased libido but states her periods are becoming irregular so she thinks she may be pre-menopausal.

Lucy’s past medical history is significant for psoriasis, psoriatic arthritis, hypoactive thyroid, and depression with a surgical history of C-section x2.  Lucy has one sister that has been diagnosed with Hashimoto Thyroiditis and her mother is a Type I diabetic.  Her recent lab work indicates hyperkalemia, hyponatremia and hypoglycemia and is also significant for elevated BUN and creatinine.  Prior to examination her vitals signs are BP (sitting) 116/64 (standing) 98/59 , HR 116, RR 18, O2 98% and Temp 98.9 F.

Scenario 3:

Maria is a 36-year-old who presents for evaluation of a cough. She is normally a healthy young lady with no significant medical history. She takes no medications and does not smoke. She reports that she was in her usual state of good health until approximately 3 weeks ago when she developed a “really bad cold.” The cold is characterized by a profound, deep, mucus-producing cough. She denies any rhinorrhea or rhinitis—the primary problem is the cough. She develops these coughing fits that are prolonged, very deep, and productive of a lot of green sputum. She hasn’t had any fever but does have a scratchy throat. Maria has tried over-the-counter cough medicines but has not had much relief. The cough keeps her awake at night and sometimes gets so bad that she gags and dry heaves.
