Consider what durable medical equipment a client will need.  Due to days to discharge, care may need to be provided in the home.  What home already has a hospital bed, a wheelchair, a bedside commode, oxygen and any other aides for care and mobility? As a case manager, the assessment of the needed equipment begins early enough that the equipment is present in the home when needed. Beyond durable medical equipment, the client may also need supplies such as wound care items, nebulizers, IV solutions and tubing, syringes, etc.  If the patient is not mobile or too debilitated to leave the home to access the needed supplies, home delivery may also need to be considered.

Each area of practice in case management (pediatrics, geriatrics, etc.) may have its own unique set of resources.  Each community is also unique in what it may or may not have, in the way of services, to offer. In general, the case manager can seek resources from this general list of possible providers: church groups, civic groups such as Kiwanis, Rotarians, employer groups, friends/relatives and local and national chapters of organizations such as the American Cancer Society, American Heart Association, etc.  In this electronic age, the internet may provide additional sources.  The United Way’s 211 Information and Referral System contains information on non-profit organizations that are present in or provide services to communities. Many cities and towns have their own websites. These can give information about community resources but also provide contacts who might have additional sources of information. Town directories are often published by the Chamber of Commerce and can provide an overview of what might be available. Community bulletin boards provide another source of information.

Technology

Case management may not be delivered in a face to face setting.  Telephonic case management is defined as the delivery of healthcare services to patients over the telephone or via the use of various forms of telecommunication such as fax, email or other forms of electronic methods. Telephone triage and the provision of health advice are common in case management. These forms of care are cost-effective and can be proactive in managing disease states with clients. Often labeled “telehealth” or “telemedicine,” both health education and medical care can be delivered via these modes. The future most likely holds in-home monitoring through full audio and visual health visits!

With electronic medical records becoming a standard in healthcare environments, this computerized technology increases the tools with which case managers can assist in monitoring patients.  Automated guidelines for care, easy access to practice standards, electronic clinical pathways, electronic care plans, electronic reminders, and automated electronic messaging have enhanced the work of the case manager. CMISs (case management information systems) are becoming key elements in assisting with the paperwork functions of the case manager.  As of 1998, these computer based systems were estimated to reduce the time that it takes to complete a comprehensive plan by one-third to one-half the time a previous plan could be generated. Thus, almost 20 years later, the case manager must be computer literate and able to manipulate these systems to streamline the workload.

Other technological systems already in place are the electronic health record (EHR), and medication reconciliation systems.
