TASK AIM:

The aim of this assignment is for you to demonstrate your understanding of community nursing concepts and principles and apply them to a context relevant to community health nursing work.

This written assignment will address course learning outcomes 1, 2 and 3.

CONTEXT:

You are a community nurse working in a community health centre. You have completed a community assessment and you have identified a number of health issues and unmet health needs in a subgroup of the local community. You plan to implement a health promotion project to address an identified health issue.

The local community health service has a grants program to fund initiatives that promote the health of local community members. Funding will be allocated to projects that focus on health promotion, disease prevention, health literacy and cultural safety in the identified community. You, and other members of the health service, will work together to develop and submit a health promotion project proposal.

TASK DESCRIPTION:

This is a GROUP assignment.

For this task you need to write a 2000 word project proposal that addresses a need for one community group (see Task Instructions below; choose one group only).

TASK INSTRUCTIONS:

1. Form a group of five students. You will work in this group for the group assessment items. You need to nominate a group leader and complete the Learning contract for group work form. This must be submitted at the end of Workshop 3 (held during the second week on-campus).

2. Choose ONE of the following options which will provide the context for your group to describe a problem / unmet community health need:

? Health and well-being for refugees from Iraq

? Cardiac rehabilitation, including lifestyle management, for Pacific Islander men who have experienced a myocardial infarction

? Women’s wellness for lesbian women living with mental health issues

? Families with adolescent children with intellectual disabilities

? Homeless young adults who live in inner city Brisbane

? Indigenous Australians living with a chronic disease and residing in a remote Aboriginal-controlled community.

3. In your proposal you must:

? Create a meaningful title for your project proposal

? Describe the aim of your project

? Describe three (3) objectives, written as specific, measurable, achievable, realistic and timely (SMART)

? Identify the problem or unmet community health need that your project will address

? Analyse the problem or unmet community health need that your project proposal will address and clearly contextualise the problem / unmet community health need for the identified group

? Describe the proposed health promotion program; this must be delivered weekly to a group over a six-week period.

? List the resources you will need to implement the health promotion program in a table

? Justify the benefits and relevance of your proposed health promotion program, including how it will address the problem / unmet community health need. Your justification must discuss relevant community nursing concepts, for example

advocacy, equity, self- management, health literacy, and engagement. Use literature to support your rationale, this can include current policies, frameworks, and relevant legislation

? Describe how the aim of the project will be evaluated (how you will assess whether you achieved the desired project outcomes).

? Ensure that you use scholarly literature (research articles, relevant Government reports and text books) that has been published from 2010 onwards

? Use the headings provided in the marking criteria to organise and present your work

? Write in the third person and use academic language throughout your proposal

? Refer to the marking criteria when writing your assignment. This will assist you in calculating the weightings of the sections for your assignment and provide a suggested word count for each section

? Include a title page with your student ID number and word count total. Do not exceed 2,000 words. Words over 2,000 will not be marked. Word limits for assessment items need to be strictly adhered to. The word limit for an assessment item includes in text citations, tables and quotations. The word limit DOES NOT include the reference list. Please note the marker will cease marking your submitted work once they have reached the allocated word limit.

? You do not need to provide a separate introduction or conclusion to your proposal

? You do not need to provide details of your health education / teaching sessions (as many of you did this when undertaking the course 2807NRS Chronic Illness Management)

ASSIGNMENT SUBMISSION:

? Your nominated group leader must submit your assignment via Turnitin as per the instructions on your Learning@Griffith course site. [Submit in the ‘assessment’ tab]

? Your nominated group leader must email a copy of your final assignment file and Turnitin submission receipt to each member of the group so they have a record of the submission.

? If your nominated group leader is unable to submit the group assignment (e.g. due to illness or unforeseen circumstances), they must nominate a proxy to upload the assignment and the campus Course Convenor must be emailed the proxy’s name.

? Your nominated group leader is responsible for downloading assignment feedback as a PDF file and emailing this file to each member of the group so they can view the assignment feedback.

GROUP DISPUTES: ? If there is conflict within your group, you must attempt to resolve this within your

group.

? If you are unable to reach a workable resolution, you must promptly notify your campus Course Convenor that there is a group dispute.

? Your campus Course Convenor, or a delegated academic staff member, will schedule a mediation meeting that all group members must attend.

? At the mediation meeting, all group members will have the opportunity to speak briefly about the issue/conflict.

? At the end of the mediation meeting, the campus Course Convenor (or their delegate), will provide all group members with meeting outcomes and/or recommendations to resolve the group conflict/dispute.

? As an outcome of the mediation process, the campus Course Convenor (or their delegate) may adjust the A2 written assignment mark for one or more members of the group (up or down).

1. Proposal title: (Less than 30 words)

? Provides a proposal title that indicates the key elements of the health improvement proposal
